There are three user profiles available for external users, the relevant profile is dependent on the type of organisation and scheme you will be reporting in relation to.

Please note: If at the time of the incident you worked in primary care and the incident occurred during a current or previous employment, choose the option ‘as an individual in primary care’. Please name the organisation where the incident occurred.
