Orphaned Record Continuity

The Orphaned Record Continuity (ORC) service securely stores patients' electronic health records (EHRs) when a GP practice closes and patients have not re-registered. This service is maintained by NHS England's Patient Record Management (PRM) team.  

About this service

1. A GP practice closes and patients may not be registered with another practice. These patients are called 'orphaned' patients. 2. While a patient is orphaned, their Electronic Health Record (EHR) transfers to the ORC service as a GP2GP message.  3. This GP2GP messages is stored securely by the ORC service. 4. If the orphaned patient registers with a new GP practice, their GP2GP message is sent to their new practice. 5. Their new GP practice can access the patient's EHR from this GP2GP message

 

When a GP practice closes, patients who do not register with another practice become orphaned patients. Their electronic health record (EHR) cannot transfer via GP2GP because the closing practice’s Organisation Data Service (ODS) code is retired.

The Orphaned Record Continuity (ORC) service mitigates this by:

  • extracting the patient’s EHR from the closing practice before system shutdown
  • storing the record securely in GP2GP message format
  • responding to GP2GP requests from a new practice when the patient re-registers

This ensures the EHR remains available for transfer, reducing gaps in clinical history and maintaining continuity of care.

This service is for integrated care board (ICBs) and commissioning support units (CSUs) involved in managing patient records when a GP practice closes or merges.

How this service works

When a GP practice closes or merges with another, in most cases, the ICB teams in coordination with system suppliers (Optum/TPP/Medicus) will move patients to other local practices. When a practice closes, ICBs will provide patients with a limited time to register themselves at another practice or designate an appropriate/approved practice for data migration. Patients who do not re-register in this time will lose their electronic health record (EHR) to date.

Practices should deduct, print and send such records to Primary Care Support England (PCSE) for storage. However, the electronic counterpart of these deducted records remains within the practice system database. Suspended records are the patient records that remain deducted and not re-registered at another practice in England. These can be re-registered via patient re-registration at a new practice. Learn more about suspended record continuity. Any records left at closure that have been deducted become orphaned when the deducting practice's ODS record closes. When a practice's ODS code retires along with it, the capability to transfer the now-orphaned record via GP2GP is no longer available.

ORC securely stores an electronic copy of the patient's record using the standard GP2GP message format. This happens before the practice’s systems close, keeping the record safe and available until the patient registers with a new GP. When a patient with an orphaned record later registers with a new GP practice, a GP2GP request can made to the ORC service, if ORC holds that record. ORC then sends the EHR to the patient's new practice and removes the patient's record from storage.

What is an orphaned record?

An orphaned record is the electronic health record of a patient who has not registered with a new practice when their GP practice closed. When the closing practice's ODS code and database are retired, these orphaned records can no longer be transferred via GP2GP.

What is a suspended record?

A suspended record is a patient record that has been deducted from a GP practice, but the patient has not yet registered at a new one.

When a patient leaves a practice, their record is deducted and sent using the PCSE print and send process. However, an electronic version of the record stays in the previous practice’s system. The Personal Demographics Service (PDS) also keeps a record of the last known registered practice.

If the patient does not register with a new practice, their record becomes suspended. This means:

  • the record is no longer active
  • it remains linked to the old practice
  • it can be reactivated if the patient registers at a new practice

If the old practice is still active, the record can be moved to a new practice once the patient has re-registered.

What happens to patient records when a GP practice closes?

When a GP practice closes, patients have a limited time to register with a new practice. 

  • If a patient registers with another practice in this time their EHR will transfer to the new practice via GP2GP. This happens for active or suspended records. 
  • If the patient does not register with another practice in this time their record becomes 'orphaned' because the remaining records are deducted.

The closing practice must print any records that have not been transferred and send them to PCSE for storage.

What happens to patient records when practices merge?

When practices merge, patient records are moved to the new practice. This is managed by the ICB and system suppliers, such as Optum, TPP or Medicus. PDS is updated for active patients.

  • If a patient re-registers during this time, their electronic health record transfers to their new practice via GP2GP. This applies whether their record is active or suspended, if the merging practice is still active.
  • If a patient has a suspended record or does not re-register, their record becomes an ‘orphaned’ record because PDS is not updated for these records.
How an orphaned record is retrieved
  1. A patient with an orphaned record registers with a new GP practice.
  2. The new practice sends a GP2GP request to the ORC service.
  3. The EHR is sent securely to the patient's new practice.

What information is available?

Who is included in the ORC service?

The service takes EHRs only for patients who:

  • were last registered at the closing or merging practice (and that practice participated in the 
  • service)
  • do not have an active registration at another practice
  • are not deceased
  • are not flagged as sensitive or invalid

What information is held?

The service usually holds full encoded electronic GP2GP record messages for each patient. Sometimes, attachments (for example, letters) may not transfer electronically and need to be printed and sent to PCSE instead.


Examples of use

ORC can ingest electronic records for deducted patients at point of practice closure or merger. These patient records can be transferred upon re-registration from ORC beyond point of originating practice closure/merger.
