CDM Lens: Subject of Care (SoC)

The Subject of Care lens is the foundational component of the CDM, focusing on the patient's entire journey through the health system.

It describes the administrative pathway alongside key clinical data elements drawn directly from the patient record. This standardisation is vital because it ensures that information needed to support direct patient care, from initial consultation to discharge, is consistent and accurately understood across all providers.

This lens provides the robust structure necessary for effective operational reporting and alignment with mandatory national-level data standards, ultimately enhancing coordinated care and patient safety.
