Impact story: PCNs and Practices Support Hub  

Purpose

This is the central workspace for 1250 Primary Care Networks across the country.

Content includes:

• Support for PCNs to develop MDT teams, including information on roles included in the additional roles reimbursement scheme.
• Tools and resources: Resources to help deliver care to patients, health and wellbeing support, and tools to improve the data available about PCNs and practices.
• Development support: Contract and policy documents, reports, training offers, career framework and regional contacts.
• Delivery support: Information and guidance to help deliver the service requirements set out in the GP contract agreement, the Network Contract DES and other relevant programmes such as flu.
• Communications: The latest news, updates, webinars and events relevant for PCNs and general practice.
