



Population Health
Following the establishment of the new NHS England, the RightCare programme has been incorporated into the wider population health focus of the organisation.
Population health data and comparisons remain a key part of NHS England’s remit. Previously produced data and ongoing updates can be obtained via Model Health System and the RightCare Tableau dashboard.
RightCare products, if still current, are displayed here under the remit of the GIRFT programme, as the lead clinical improvement programme.
- Toolkits
These toolkits support systems to understand the priorities within a given topic area and recommend the key actions to take to deliver change locally. They provide an opportunity to self-assess and benchmark current systems’ service delivery and to explore opportunities for improvement. This supports improving population health and making optimal use of our collective investment in the health of our population.

- Scenarios
These scenarios put the person at the centre of the story. They use fictional service users to show the difference between a suboptimal – but realistic – pathway of care, compared to an optimal one. Where possible, the costs of care are highlighted, not only financial to the local health and care system, but also the impact on the individual and their family. Scenarios support local health and care systems to think strategically about designing optimal care for people (and their carers) with long term or high impact conditions.
