What you need to nominate

  1. Nominee’s personal details

    You’ll need your nominee’s name, age, address and contact details. You’ll also be asked for your nominee’s gender, ethnicity and nationality to help us with our diversity monitoring.

  2. Nomination details

    You’ll need to write a detailed description explaining why you’re nominating them. Read the guidance on how to write a nomination (opens in new tab).

  3. Evidence of your nominee’s contribution

    Any documents which provide evidence of your nominee’s contribution. For example, newspaper clippings or letters.

  4. Supporting letters

    We need two or more letters of support which endorse your nominee’s contribution. These should come from people who are familiar with your nominee’s services.

    Supporters can address letters to you as the nominator or to the Honours and Appointments Secretariat.

  5. Your contact details

    We need to collect some information about you in case we need to contact you.
