Crynodeb
The post holder will provide direct nursing care predominately in the patients home, and on occasions in other settings such as Community Hospitals, Residential & Nursing Homes and Eden Valley Hospice. The post is 18 hours per week. This is 2 nights per week. The post holder will provide nursing care, emotional, social & psychological support to individuals with palliative care needs, their families & carers. This involves regular and extended involvement of assessing fluctuating needs and the stress of caring for the dying patient and their families. To take part in an on call rota covering hours outside standard service times.
