Crynodeb
The focus of this role will be on completing the following main duties; Memory Assessments within clinic or patients home environment, dependent on patient need. Timely diagnosis, following close joint working with Consultant psychiatrists. Pharmacological Treatment, as per guidance from Consultant psychiatrist. As appropriate depending on profession. Post diagnosis support and sign posting. Monitoring and reviewing of patients started on treatment. Contribution to duty system. Assessment of risks at home and support to address/ mitigate utilising MDT approach. Please see job description for full details.
