The post holder will carry out mental health care and support to older people with mental health problems in the community in accordance with a laid down Programme of care as designed by the multi-disciplinary team. The support worker will be working under the direction of qualified nurses and social workers and encourages independence within the service users social network. The post holder will regularly work alone without direct supervision for part of the span of duty. Support workers will have close working links and work in a coordinated manner with the members of the multi-disciplinary team, as well as Day Centre staff, Home Care staff and any other voluntary or statutory agencies involved with service users in their care. Job Responsibilities: o To appraise senior staff of service users progress and observations of changes in their mental health condition and ensure that these changes are documented in service users notes.o Assist in the implementation of care ensuring service users rights to privacy, dignity, choice, independence and fulfilment are adhered to at all times. o Participation in multi-professional assessments and reporting back to other care agencies. o Providing support to service user and/or carer in accordance with a care plan, (maintaining social networks, maintaining a safe environment, communication/ liaison with other service providers, reassurance, and emotional support persuading service user to accept support). o Participation in multi-professional reviews and Care Programme Approach/Care Management meetings. o Report any changes in circumstances in respect of the service users care to the agencies who are or may need to be involved in the service users or carers support network improving the quality of life, rehabilitation and independence of service users and their carers. o To ensure that all care services provided are offered without prejudice regardless of age, creed, gender, and ethnicity within their sphere of responsibility. o To develop awareness of service users cultural and ethnic needs to facilitate appropriate support and care. Working Relationships and Communication Requirements of the Job Working relationships As the post holder will be working within the community most of the time, they will need to be establishing and maintaining positive relationships with people, agencies, charities and other providers whom also provide services to Older People in that community area. These relationships are vital and team members should bear in mind that they will be representing HPFT Close working relationships should be developed with other parts of HPFT notably Crisis teams, Intensive Outreach Team and SMHTOPs Communication requirements The post holder will Be able to receive and give clinical information verbally and in writing from/ to those receiving care, their families/carers and professional colleagues. Demonstrate a high level of interpersonal skills, self-awareness, and empathy in all communications. Ensure adherence to all aspects of patient confidentiality, documentation and record keeping according to Trust and national guidelines. Will have the ability to communicate clearly and sensitively in complex situations. Will have the ability to develop and maintain good working relationships with colleagues within the all agencies whom they encounter to the benefit of the client and the service. Clinical Responsibility The post holder will o To demonstrate clear communication skills, both verbal and written. o Observe and record outcomes of care delivered. Reporting to the Care Coordinator in a timely manner depending on urgency of situation. o To assist where appropriate the establishment and running of carers support and user groups within a variety of community settings. o To ensure effective and efficient use of resources. o To maintain collaborative working relationships with colleagues to ensure effective teamwork. o To report incidents, complaints and compliments to the Care Coordinator/ Team Manager o To assist service users to live independently in the community and to support their carers and to assist the multi-professional team in assessing, planning, implementing, and evaluating care. o To comply with all policies and procedures of the Hertfordshire Partnership NHS Trust. o To attend individual and group supervision within the multi- disciplinary team.o To participate in individual development and performance reviews as required. o To enter data on information systems as required. o To participate in multi-professional/multi-agency training and education courses as agreed with the team manager. o To attend all mandatory training and education courses as required. Leadership and Staff Management Responsibility The post holder will participate in teaching sessions to the Team at Team Meetings and other organised events. Financial responsibility the post holder will Report any activity that may be fraudulent Be responsible for the completion of own extra duty and expenses claims Service Development and Improvement The post holder will Ensure that all policies applicable to SMHTOP are implemented To comment upon and propose changes to working practices and procedures relevant to SMHTOP. Where requested by the Team Manager, implement changes. To propose ideas for, and if accepted, complete audits relevant to the improvement of the teams clinical practice Analytical and Judgmental Skills The post holder will Have an understanding of the Mental Health Act, Mental Capacity act, National Dementia Strategy and National Service Frameworks for Older People and Mental Health. The post holder will also understand the Safeguarding Adults procedures. Assist the Care Coordinator to make judgements where the situation is not straightforward such as those where information is limited or difficult to obtain. Assist the Care Coordinator to make judgements as to the best course of action where there are a range of options available. Planning and Organisational Skills The post holder will Demonstrate the ability to prioritise and organise case/ workload on a day-to-day basis. Ensure that the client is always given high quality care. Where necessary request assistance from colleagues. Physical Working Conditions and Environment The post holder will Need to be able to drive to patients homes/ meetings /training off site and occasionally as required cover for colleagues on other sites. Be able to input information using computer keyboard. Be able to restrain people as necessary and maintain training for such as per HPFT policies/procedures. Be able to work in an environment that is at times extremely busy and demanding. Information Resources The post holder will Be expected to maintain a record of hours worked and to time owing record monthly. Maintain up to date electronic records of people that the post holder has had contact with. Maintain an accurate electronic diary for the purpose of transparency and lone working. Will follow the lone working policy Audit activity as required.
