triage all incoming referrals to the service Provide specialist nursing advice and support to patients, families, carers and other Healthcare Professionals following diagnosis and through treatment Ensure continuity of high quality evidence based nursing care, including assessing health and nursing needs of patients, families and carers. Identify and initiate appropriate effective care, including; -Managing a patient case load -Ordering and performing diagnostic tests -Making referrals to appropriate specialities for onward treatment as required -Admitting and discharging women with continence needs within agreed protocols Undertake outpatient clinics for nurse-led pessary / urodynamic diagnostics / pelvic floor and incontinence clinic / support group / post-operative follow up clinic. Ensure the BSUG database is kept up to date to enable accreditation of service. Work within UKCS Pessary Guidelines and urodynamic standards. Accept responsibility for own patient caseload, ensuring all patients have an accurate plan of care, which reflects the assessment undertaken and incorporates the issues and recommendations made, ensuring clear documentation in the patients records, including outcome letters sent to patients GP surgery. Perform urodynamic studies and potentially other invasive procedures (such as flexible cystoscopies) independently and in collaboration with the urogynaecology clinicians, advise on appropriate treatment plans To be actively involved in inpatient care, liaising with ward staff to ensure high-quality nursing care including support of timely discharge of post-op patients and ensuring follow up care to appropriate urogynae clinic is arranged. To follow up patients as appropriate and ensure all relevant information is available to enable patients to make an informed choice about their treatment. To attend, coordinate and contribute to multi-disciplinary team meetings, ensuring all relevant patients are represented. To deliver a seamless service through the development of enhanced multi-disciplinary team processes and communication. To participate in the identification and development of clinical protocols and strategies to enhance both the continuity and standard of urogynaecological care whilst ensuring equity of access to the service. Demonstrate clinical skills and expertise to all grades of staff, including junior medical staff. Review and evaluate care given by other team members, ensuring that realistic goals are set to achieve optimal outcome. Act as a role model and clinical leader, demonstrating knowledge and research-based best practice. Identify safeguarding and vulnerable adult issues which affect the rights of individuals Develop appropriate clinical skills related to specialist practice- such as pessary insertion, urodynamics, continence assessment, menopause assessment To work with key staff members to deliver nurse-led assessment and outpatient follow-up services, providing detailed baseline patient assessment and education and support to patients and their families. To work with the multi-disciplinary team to develop, implement and evaluate integrated care pathways and systems of multi-disciplinary team documentation To liaise with local community, social services and voluntary organisations in order to develop existing services for clients, carers and health professionals. To involve service users actively in providing feedback of their experience of the current service and suggestions for improvements. To participate in the development of appropriate patient group directives to facilitate and enhance the discharge process. To ensure all patients are fully informed of the risks and benefits of proposed procedures and treatments, contributing to the process of securing informed consent. To produce and provide to all patients relevant and accessible written information regarding their treatment in accordance with Trust standards. To prescribe safely and appropriately in line with NMP formulary (if held)
