Crynodeb
We are seeking a motivated and compassionate Mental Health Support Worker to join our Home-Based Treatment Team (HBTT). The role involves supporting individuals experiencing acute mental health crises within their own homes as an alternative to hospital admission. You will work as part of a multidisciplinary team, providing person-centred care, promoting recovery, and supporting service users to maintain independence and safety in the community.
Provide high-quality, person-centred support to individuals experiencing acute mental health crises within their own homes and community settings.
Deliver and support agreed therapeutic and psychosocial interventions as part of care plans, under the direction of qualified practitioners.
Work collaboratively with service users, carers, and families to support recovery, promote independence, and maintain safety.
Contribute to the assessment and ongoing monitoring of mental health needs, including identifying and reporting changes in presentation or risk.
Support the implementation of crisis and risk management plans in partnership with the multidisciplinary team (MDT).
Build positive, therapeutic relationships with service users and those important to them, including carers and family members.
Maintain accurate, timely, and professional clinical records in line with Trust standards.
Work closely with colleagues across the MDT and wider services to ensure coordinated, effective care delivery.
