Summary

22.5 hours per week (3 days per week)

This role is office-based; however, it also offers some agile or remote working.

The Adult Social Care Transition Team are seeking to appoint a dedicated part time qualified Social Worker to be part of our dynamic, supportive and fun team.

We are a group of passionate and motivated staff, who support young people with additional needs as they move from children’s services through to Adult Social Care, ensuring a smooth and person-centred transition into adulthood.

This transition can be an exciting time with lots of new opportunities, but equally it can also be a worrying time for young people, when they move on from familiar people and places and the role of the transition social worker is paramount.

About the role

As part of the Transition Team, you will work with young people aged 14-25, their families, carers, and other partner agencies to assess needs, develop care plans, and coordinate services that promote independence, wellbeing and positive outcomes.

You will play a key role in helping individuals navigate important life changes, while ensuring compliance with relevance legislation such as the Care Act 2014.

Key Responsibilities

Undertake strengths-based assessments and reviews
Develop and implement person centred care and support plans
Work collaboratively with education, health, and children’s services colleagues
Support safeguarding responsibilities where required
Promote independence, choice and inclusion for young adults
