Summary
Undertake clinical assessments and prescribe in line with best practice. Develop and review person-centred care plans. Deliver BBV testing, immunisation (including Hep B) and harm reduction interventions. Liaise with pharmacists and GPs to support shared care arrangements. Maintain accurate clinical records and contribute to audit and governance processes. Participate in CPD, supervision and service improvement activities.
