As a Community Continence Assessor (HCA), you’ll help deliver care to people in their own homes and get to work with inspiring, skilled teams whilst you do it. The Continence Health carer will contribute to the efficient running of the District Nursing Team and participate in decision making with regard to providing holistic patient care. They will work under the supervision / direction of the District Nursing Sister / Charge Nurse or nurse in charge to ensure the delivery of high quality/ individualised patient care.



• Assist the District Nursing Sister in achieving a high standard of care that promotes the health and wellbeing of patients in their own home.

• Prioritise workload appropriately with due regards to the requirements of patients/ carers and the District Nursing Teams.

• Be responsible for ordering and maintaining stock and continence equipment.

• Work as a member of the District Nursing Team, contributing to the overall work of the team within the boundaries of their role and attend meetings as required.

The “must haves” for this role:

1. Hold a full UK Driving Licence and have use of a car for work purposes

1. A passion for community nursing, that matches our passion to provide excellent healthcare for patients within their own homes

1. Willing to undertake training to fulfil the requirements of these role
