Job Responsibilities Responsible for coordinating and delivering digital inclusion support activities at planning, development and delivery stages to accelerate and maximise digital inclusion across South Notts PBP services and communities. Build and maintain networks through engagement with practice teams and the wider community. Utilise influencing and communication skills to educate others on the benefits of digital services, whilst recognising exclusion barriers and challenges. Upskill and improve confidence in using digital services through tailored support and training of members of the public and staff. Coordinate training opportunities for South Notts PBP staff and local third sector organisations. Utilise strong communication skills to deliver presentations to a wide range of audiences. Contribute to the positive promotion of digital health services available to members of the public (NHS App and other / integrated services) for improved management of health, care and wellbeing. Work with local colleagues to identify key cohort groups to target activity, reducing the barriers to digital inclusion for those that experience the greatest health inequalities and ensuring positive impact on patient outcomes. Responsible for monitoring progress against agreed objectives and targets, ensuring that reporting takes place. Where necessary evaluate own work and impact relating to specific activities undertaken and use this information to develop services as appropriate. Develop communication materials that can be included within newsletters, social media posts and practice websites, showcasing the benefits of digital healthcare services and the impact of the roles work. Responsible for supporting South Notts PBP in having health literate information on relevant websites about digital health services and the support available through the Co-ordinators own role, ensuring it is accurate and kept up to date. Complete mandatory / other training to support the knowledge of the post-holder. Act in accordance with the policies and procedures of the organisation. Work closely alongside other staff on wider digital transformation projects aligned to South Notts PBP / ICS priorities. Work in partnership with a vast network, including: the local digital inclusion workstream, local digital health champions network, CVS Organisations, national NHS App team and NHS App Ambassadors by sharing and acquiring information, knowledge and opportunities. 1. Learning and Development Undertakes any training required to develop or maintain their proficiency within the service area and demonstrates competence within professional requirements Evaluates own practice and identifies areas of development by setting appropriate objectives via the appraisal process Is committed to delivering and sharing learning opportunities with students, team members and other agencies to develop self and others 3. Partnership and Team Working Actively works towards developing and maintaining effective working relationships within the team and the wider stakeholder group Responsible for communicating information to the team, providing advice and support as necessary; this can be challenging and complex in its content i.e. Implementing organisational and/or services changes Explores the potential for collaborative working and takes opportunities to initiate and sustain such relationships. Responsible for investigation of complaints within own area if necessary Working with internal and external service users in line with organisational policy and guidelines Actively contributes to the programme team supporting including communication with external providers and other agencies; acting as a credible source of information for other agencies/professionals involved as required. 4. Innovation and Quality Works with managers and colleagues to continually improve the quality-of-service delivery. Acts as an advocate for service users recognising the boundaries of their knowledge; supporting them with their needs whilst liaising and signposting on to other services/agencies as required 5. Freedom to Act Post holder will work with close supervision within well-established procedures and has standards and results to be achieved 6.
