Crynodeb
The Young Person's Triage and Outreach Worker will sit within the social prescribing team as a first port of contact for referrals from professionals for young people, assessing initial needs, providing early signposting, advice and onward referrals to other services. You will also link in closely with external young people's support services. Main duties and responsibilities: 1. Take referrals for young people (aged 12-17) from GP practices within Chesterfield & Dronfield PCN and triage appropriately to assess suitability for onwards support through social prescribing or other available services. Approximately 3 days per week. 2. Take referrals for the adult Social Prescribing service approximately 2 days per week, providing extra support to cover holidays or sickness when required. 3. Build and maintain a database of support options for young people across Chesterfield and Dronfield, creating good relationships with referral partners. 4. Manage a dynamic short-term caseload of patients with a view to connecting them to relevant services based on "What matters to them". 5. Provide personalised support and early signposting to individuals, their families and carers to take control of their wellbeing and improve their health outcomes. Supporting them with a wide variety of non-medical related issues. 6. Develop relationships by giving people time and focus on what matters to me. Take a holistic approach, based on the persons priorities and the wider determinants of health. 7. Co-produce a personalised action plan of first steps to improve health and wellbeing, signposting or referring people to community groups and statutory services. 8. Work closely with the existing PCN social prescribing service to ensure support is provided for the whole family where needed, acting as a source of information and advice for relevant signposting and referral routes. 9. Work as part of a multi-disciplinary team, organising MDTs and CSTs where appropriate, and inviting relevant agencies to attend. 10. Liaise and communicate with Patients, Carers, Advocates, Health and Social Care professionals, voluntary sector organisations and stakeholders involved in the wellbeing of patients and communities. 11. Ensuring that work is delivered in a timely and effective manner, the role will require managing and prioritising your own workload, in accordance with the needs, priorities and any urgent support required by individuals at triage. It is vital that you have a strong awareness and understanding of when it is appropriate or necessary to refer people to other health professionals based on level of need. 12. Build relationships with staff within the local PCN, attending relevant meetings, becoming part of the wider network team. 13. Work in partnership with local agencies and PCN's and represent the service at internal and external meetings to raise awareness of social prescribing, strengthen referral pathways and support integrated working across health, social care and the voluntary sector 14. Work sensitively with people, their families and carers to capture key information, enabling tracking referrals into the service and outwards to other services. 15. Work closely with the Social Prescribing Lead to analyse referral trends, community insight and population need to identify gaps in provision, and contribute to the strategic planning and development of the service by providing evidence-based recommendations to promote the service, and develop and maintain smooth referral pathways for internal and external referrals, and to provide this data to management. 16. Take an active role in quality improvements and innovations, e.g. audits, significant events analysis and development of protocols and new services, including designing and testing new approaches to outreach and triage in response to identified community and service need. 17. To undertake general administrative duties to support the role. 18. Take a lead role on specific projects or workstreams and provide guidance or support to colleagues where appropriate. 19. and any other duties commensurate with the role as advised by your line manager.
