Crynodeb
We are recruiting two Health and Social Care Assistants to join our community care team. These posts are essential in meeting service needs across early and late shifts, supporting people in their own homes to maintain dignity, wellbeing and independence. Flexibility is key, as the service runs across two shift patterns.
What you’ll do:
• Provide personal care and practical daily support in line with individual care plans
• Support with meals, hydration and taking prescribed medication
• Observe and report changes in wellbeing to the wider team
• Keep clear, accurate records using electronic systems
• Work flexibly across early and late shifts to meet service needs
What we’re looking for:
• A caring, respectful approach and good communication skills
• Experience in care or transferable skills, with a willingness to learn
• Ability to workboth shift patterns: 8am–4pm and 2pm–10pm
• Availability to work30 hours per week across five shifts, depending on service need
