University Hospitals Dorset in conjunction with the Motor Neurone Disease Association, are seeking a dynamic, compassionate, and highly organised professional to join our team as aCommunity Care Coordinator for Motor Neurone Disease (MND).

This is a pivotal role focused on delivering high-quality, coordinated care for people living with MND and supporting their families across community services. You will act as a key point of contact, ensuring seamless care pathways, improved patient experience, and timely access to multidisciplinary support.


This is an exciting opportunity to play a crucial role in the care coordination of patients with MND. This post will be primarily based in the west of the county but will work closely with colleagues in the east to support providing equitable care across the county.

Interview Date: 18th June 2025
• Act as the primary coordinator for patients diagnosed with MND within the community setting
• Work closely with the multidisciplinary team (MDT) including neurology, palliative care, therapy services, and primary care
• Support the development and delivery of personalised care plans
• Ensure timely coordination of equipment, services, and interventions
• Provide expert advice and emotional support to patients, carers, and families
• Facilitate communication across acute, community, and voluntary sector services (e.g., MND Association)
• Lead on service improvement initiatives and contribute to pathway development
• Monitor outcomes, maintain accurate records, and support reporting requirements


UHD are investing, developing and transforming Trust services in line with the New Hospital Programme. As part of this, some services may move site this year or next, either temporarily or permanently. Recruiting Managers will be happy to answer any service-specific questions at interview.

If a role or service relocates as part of a planned move, excess mileage will not be reimbursed. Travel from home to the new work base will be classed as a normal commute. Any other changes will be managed under Trust or national terms and conditions.
