Summary
Offering specialist nursing care, support and monitoring to patients with cardiovascular disease and heart failure Delivering cardiac rehabilitation both face to face in classes and home based (heart manuals and/or digital). It involves support to optimise treatments prescribed following diagnosis and monitoring effects. It involves education and support around lifestyle issues and risk reduction Provides day-to-day supervision and/or clinical supervision to others. Undertakes specialist assessment, planning, implementation and evaluation of clinical care for caseload of patients/clients with cardiovascular disease and heart failure. Provides specialist advice to patients/clients/carers Is an accountable practitioner. Maintains records as autonomous practitioner. Skilled at assessing and managing risk. The job entails independent travel around the Easington/Seaham area with possible need to support other teams across the CDDFT footprint.
