Crynodeb
You’ll work as part of a supportive multi‑disciplinary team, alongside registered clinicians such as occupational therapists and physiotherapists. The role focuses on delivering safe, high‑quality care through assessment, treatment programmes and health promotion activities. You’ll build trusted relationships with service users, carers and colleagues, helping people stay active, independent and engaged in their care.
What you’ll do:
• Deliver agreed therapy and rehabilitation programmes under clinical supervision
• Support assessments and provide care in community and home settings
• Manage a defined caseload and contribute to person‑centred care plans
• Motivate and support people to take part in healthy living activities
• Record clear, accurate clinical information and share updates with the team
• Support and guide support workers and less experienced colleagues
What we’re looking for:
• A relevant Level 4 qualification, foundation degree, or equivalent experience in health or rehabilitation
• Experience working with people in a clinical or care setting
• Good communication skills, with a calm and respectful approach
• Ability to work independently while knowing when to seek support
• Commitment to safe practice, learning and team working
