Key Responsibilities Will Include: Provision of chronic disease management, for patients with Diabetes, COPD, Asthma, Cardiovascular disease and Hypertension Domiciliary care and care home cover Undertake assessments, reviews and care planning Maintain accurate, contemporaneous clinical recordsWork collaboratively with multi-disciplinary teams and General Practitioners Participate in quality improvement and audit activities Other duties will include health screening, vaccines and injections as appropriate health promotion activities Practice Infection Control lead
