Crynodeb
The role holder will be an experienced Health Care Assistant currently working in Primary or Community Care. They will be expected to work autonomously within the nursing team to deliver excellent patient care, in the community. The post holder will be expected: To work autonomously, to provide holistic care to patients. Plan visits ensuring they are undertaking in a timely manner in line with practice protocols. Review patients following practice protocols, identifying areas for support offering advice, guidance or escalation as needed. To seek out opportunities for practice development within their own scope of practice. To support the learning and development of colleagues, students, professionals and patients within the surgery and the Primary Care Network. To embed new/changing policies, protocols, and procedures within the nursing team to meet the needs of the practice. To support the nursing team to meet local and national targets relating to patient care. To hold a full, clean driving license and appropriate car insurance. Responsibilities within Practice Patients and their families have the right to be treated fairly and be routinely involved in decisions about their treatment and care. The practice is committed to patient confidentiality upholds statutory legislation in relation to this. The practice is a training practice and there is an expectation that all team members will provide a support role to various degrees. The practice is part of Wakefield West Primary Care Network and works closely with other practices and colleagues within the network. All members of staff are expected to uphold this collaborative working. The practice is committed to supporting and promoting opportunities for staff to maintain their health, well-being and safety. You have a duty to take reasonable care of health and safety at work for you, your team and others, and to cooperate with employers to ensure compliance of health and safety requirements. All personnel are to comply with the Health and Safety at Work Act 1974, Environmental Protection Act 1990, Environmental Act 1995, Fire Precautions (workplace) Regulations 1999 and other statutory legislation. Furthermore, it is expected to be vaccinated in line with current guidance. Staff are required to dress appropriately in the workplace, in uniform if applicable. Consideration to infection prevention and control should be made for clinical roles. The practice has a no-smoking policy. Smoking is not permitted on site and staff should not smoke in identifiable uniforms or with name badges visible. Support can be offered to members of staff who wish to stop. Car parking is available on site and free of charge. An Enhanced Disclosure from the Criminal Records Bureau will be requested in the event of a successful application in line with the Rehabilitation of Offenders Act 1974.
