Summary
You will demonstrate empathetic and professional communication with patients and their significant others, consistently providing high-quality personal care and support to enhance overall wellbeing. Contract: Permanent Working pattern: Monday to Sunday, including bank holidays Shift pattern: Shifts rostered between 7:30am and 8:00pm Salary: £25,272 per annum Base: Community-based across the West Midlands, with a hospice base. The postholder will primarily provide care in peoples own homes, with occasional support on the inpatient unit. What we are looking for: Recent experience providing care within a health or social care setting, including an understanding of palliative and end-of-life care. Experience of delivering care in a community setting and contributing to person-centred assessments. NVQ Level 2 in Health and Social Care, or an equivalent Level 2 qualification, is desirable but not essential. Person-centred and compassionate when caring for the person and those important to them. Ability to develop and evaluate care management plans that meet the complex needs of the person, and those important to them. Emotional resilience and the capacity to work independently. Possession of valid driving license, access to a car, and be willing to drive up to 25 miles.
