Learning from SPADs using detailed causes data.

Safe Insights is Great Britain’s national system for sharing high‑quality rail safety event data. Since 2021, detailed causes data for Signals Passed at Danger (SPADs) has been recorded in Safe Insights. This data helps us understand why SPADs occur and how the railway system can better manage risk.

This webinar presents an analysis of that data, focusing on the 10 Incident Factors. These describe how systemic issues such as workload, resource, equipment design, and competence can influence events.

Passenger journeys are largely safe but residual risks remain, making effective mitigations and collaborative sharing of good practice essential to reducing accidents and maintaining passenger confidence. This webinar will outline key Passenger Operations workstreams, highlighting recent developments at the Platform Train Interface (PTI), including the PTI Strategy and updates to the PTI Risk Assessment Tool (V4).
