The post holder will support the Named Nurse for Looked After Children in delivering a high-quality, effective, and compassionate health service for Looked After Children, Care Leavers, and other vulnerable groups. They will contribute to improving health outcomes by ensuring statutory requirements are met and that children and young people receive timely, holistic, and child-centered care. The role includes undertaking statutory health assessments in a variety of settings, including home, clinic, and community environments, and developing, implementing, and reviewing individualised health care plans. The post holder will manage their own caseload, ensuring effective prioritisation and coordination of care in partnership with children, young people, carers, and professionals. The post holder will work collaboratively within a multi-agency framework, attending and contributing to professional, strategy, and review meetings, and liaising with colleagues across health, social care, education, and other partner organisations. They will provide specialist advice, guidance, and support on the health needs of Looked After Children and Care Leavers, including complex and sensitive cases. A key element of the role is to promote evidence-based practice and contribute to the development, implementation, and review of policies, procedures, and clinical guidelines in line with local and national standards. The post holder will support safeguarding practices and ensure that children and young people remain at the centre of all decision-making. The role also involves delivering and supporting training for health professionals, foster carers, and partner agencies, helping to raise awareness and improve knowledge of the health needs of Looked After Children. The post holder will contribute to service development, quality improvement initiatives, audit, and performance monitoring to ensure continuous improvement of the service. In addition, the post holder will demonstrate effective team working, act as a positive role model, and support colleagues through supervision, advice, and shared learning.
