Key Responsibilities The Advanced Nurse Practitioner will: Work autonomously within their scope of practice and be accountable for their professional actions, demonstrating a high level of clinical judgement and decision-making. Deliver advanced clinical nursing care using expert knowledge and skills, providing holistic, patient-centred care to the registered practice population. Assess and manage patients presenting with a wide range of acute, non-acute, and long-term conditions, including taking comprehensive histories, performing clinical examinations, developing differential diagnoses, and implementing appropriate management and treatment plans, ensuring suitable follow-up arrangements. Initiate referrals to other healthcare professionals and services within primary and secondary care as appropriate, and arrange hospital admissions when clinically indicated. Undertake telephone triage and provide clinical advice remotely, ensuring safe and effective patient management. Prescribe medications in accordance with current legislation, local policies, and clinical guidelines. Maintain accurate, contemporaneous, and comprehensive clinical records within the EMIS Web system, in line with professional and organisational standards. Carry out medication reviews and contribute to effective medicines management. Request, interpret, and act upon diagnostic tests and investigations in line with patient needs and best practice. Participate in clinical audit, quality improvement initiatives, and service development. Actively contribute to multidisciplinary team working, including participation in clinical meetings and practice discussions.
