With around 33% of our population living in some of the most deprived areas in England, this role offers a genuine opportunity to reduce health inequalities and ensure people receive the right care, at the right time, in the right place.
This is a rewarding, autonomous Band 6 role where your clinical judgement, case management skills, and system understanding really matter. As a Commissioning Mental Health / Learning Disabilities Nurse, you will:
• Assess individuals with functional mental health needs in line with the National Framework for NHS Continuing Healthcare and Funded Nursing Care, ensuring full compliance with the Mental Health Act
• Manage and review complex packages of care, commissioning services that meet identified health outcomes
• Lead on Section 117 aftercare funding applications and oversee the
commissioning of appropriate aftercare packages
• Undertake timely reviews, including regular Care Programme Approach (CPA) reviews, in line with service KPIs
• Provide robust case management for individuals eligible for CHC or other NHS-funded care
• Work closely with acute hospitals to support Discharge to Assess pathways, ensuring safe, timely transitions of care
• Champion a consistently high standard of compassionate, person-centre

• Has experience in Section 117 aftercare, care commissioning, and case management
• Is comfortable working with complex needs and navigating multi-agency environments
• Communicates clearly and professionally across discipline
• Is adaptable, organised, and able to prioritise in a fast-paced setting
• Is committed to partnership working and improving health outcome
