Arrange meetings Take minutes in meetings Ensure actions from meetings are completed in a timely manner Send out meeting agendas and associated papers out documents Act as a contact point for the PCN Send/receive e-mail's on behalf of the Clinical Director and PCN Strategic Manager where appropriate Complete actions for Clinical Director and PCN Strategic Manager where appropriate Keep Teams groups and documents updated Provide admin support to other leadership positions within the PCN where required Work closely with the Social Prescribers and Health and Wellbeing Coaches Supporting the PCN leadership team to look at population health intelligence to help proactively identify and deliver personalised care Work with the GPs and other primary care professionals within the PCN to help support them in identifying and managing a caseload of patients, and where required and as appropriate, refer people onward to other health professionals within the PCN Raise awareness within the PCN of shared decision-making and decision support tools Raise awareness of how to identify patients who may benefit from shared decision making and support PCN staff and patients to be more prepared to have shared decision-making conversations. EMIS Have a good understanding of how to use EMIS to support with adding rotas, making changes to templates and organising/booking patients where required across the PCN. Key Working Relationships and Communications PCN Clinical Director, Strategic Manager and Operations Manager Practice Managers General Practitioners and other practice clinicians SWGP Director of Planning and Delivery ICB Patients & public stakeholders, including patient participation groups Relevant representative committees (e.g. LMC, LDC, LOC, LPC) Healthcare providers including NHS, third sector and voluntary providers Review of role A personal development plan will be agreed each year with the post holder to further develop the skills and knowledge required to carry out the responsibilities associated with this role.
