Crynodeb
As a Specialist Nurse, you will demonstrate the ability to work both independently and collaboratively, supporting the care of patients with stomas in hospital and following discharge across Southern Derbyshire.
You will work in partnership with a diverse multidisciplinary team and therefore will need strong, compassionate communication skills, with the ability to engage effectively with patients, families, carers, and colleagues.
Under direction of the Lead Specialist Nurse deliver care to defined case load of patients requiring specialist care. You will act as the patient’s advocate maintaining privacy and dignity and provide support, advice and education to patients/significant others in the management of
their care.
• Post operative teaching & education with patients, including dietary advice
• Troubleshooting for new and established stoma patients
• Onwards referrals for psychological support
• Specialist input such as dermatology and IBD
• Routine reviews as part of post op pathway
• Telephone triaging
• Responding to advice and guidance requests
• Must hold full driving licence and have use of own car
