Crynodeb
Lead a team of registered professionals to deliver the core functions of AACC. Provide expert clinical oversight of the application to the National Continuing Care Frameworks including eligibility decisions, disputes, appeals and PuPoCs.Where required support as a key leader in the delivery of national programmes such as Transforming Care and Inpatient Quality Transformation, LeDeR, S117 Aftercare, Neurorehabilitation and Complex Care. Where appropriate to the role ensure individuals are offered C(E)TRs in line with national guidance and within required timeframes, engaging with people with lived experience, their families, and carers to embed effective C(E)TR delivery within the local area. Attend MDTs and Predischarge meetings to support discharge for people with complex needs to ensure that they are supported appropriately in the appropriate placement that meets their needs. Participate in the operational aspects of the Learning from Lives and Deaths - People with a Learning Disability and Autistic People (LeDeR) programme and carry out LeDeR reviews as and when required, in accordance with nationally agreed standards and key performance indicators. Directly supervise the team in relation to Human Resources, e.g., absence management/sickness, appraisal and work performance. Ensure that assessments and reviews are carried out promptly according to national specifications.
