Crynodeb
To contribute to improving health outcomes in patients with long-term conditions as a member of both the pharmacy team and the wider practice multidisciplinary team. To provide specialist knowledge and advice on pharmaceutical matters, including cost-effective prescribing, medicines optimisation, and the implementation of clinical guidelines for disease treatment. To also provide evidence-based education and training for colleagues in partnership with the practice. To practice as an Independent Prescriber, undertaking face-to-face consultations and managing patient care within scope of practice. To provide advice, guidance, and support for patients presenting with self-limiting conditions. To work collaboratively to improve communication regarding medication-related issues between the practice and other healthcare providers. To contribute to the implementation and ongoing development of safe and efficient repeat prescribing systems. The practice pharmacy team forms part of the wider multidisciplinary team and currently consists of 3 Pharmacists, 2 Pharmacy Technicians, and 3 Prescribing Clerks, all working collaboratively to support safe, effective, and patient-centred care.
