Main Duties & Responsibilities Heading Duty/Responsibility Clinical Act as Care-Coordinator for appropriate clients under the Care Programme Approach (CPA). Be a full member of the multi-disciplinary team (MDT). Effectively manage own caseload accepting referrals from agreed agencies in accordance with agreed operational procedures. Assess, care-plan, provide interventions and evaluate and record community mental health interventions delivered to clients on own caseload. Undertake formal mental health assessments of new clients within own caseload. Undertake assessments of clients as part of the EIT duty system in accordance with agreed operational procedures. Contribute to the service duty roster for their team locality To have knowledge of and contribute to the team adherence to the NHS England Access and Waiting Time Standards for EIP. To manage discharges from the service and transfers to other services as appropriate for service users who are not accepted for the full First Episode Pathway service following assessment. To undertake risk assessment and risk management for individual service users and to provide advice to other professionals on acute and chronic aspects of risk assessment and risk management. To work as part of our integrated MDT, to attend daily meeting to feed back the outcomes of assessments undertaken, and to participate in a clinical decision-making process to determine the most appropriate management plans for all service users who have received an assessment. To have a good knowledge of the required documentation for case management, and to ensure compliance with any associated performance standards. To maintain a high standard of clinical record keeping, including electronic data entry, and recording, report writing and the responsible exercise of professional selfgovernance in accordance with relevant professional code of practice and Trust policies and procedures. To liaise professionally and sensitively with other involved parties in order to obtain further information pertinent to the holistic assessment process. To provide advice, guidance and consultation to peer and junior professionals within the service, both individually, and by attendance at team referral and review meetings . Teaching, training and supervision The post holder will receive regular clinical and managerial supervision. To remain informed of current research and guidance to support own professional work and to contribute related knowledge to the MDT. General To contribute to the development and articulation of best practice across the service, taking part in regular professional supervision and appraisal and maintaining an active engagement with current developments in the field of Early Intervention for Psychosis. To maintain up to date knowledge of legislation, national and local policies, and issues in relation to both the specific client group and mental health. To maintain high standards of quality in corporate and clinical record keeping ensuring information is always recorded accurately, appropriately and kept up to date. To ensure their day-to-day activities embrace sustainability and reduce the impact upon the environment by minimising waste and maximising recycling; saving energy; minimising water usage and reporting electrical faults, water leakages or other environmental concerns to the facilities department or their line manager. Take reasonable care of the health and safety of yourself and other persons To contribute to the control of risk and to report any incident, accident or near miss To protect service users, visitors and employees against the risk of acquiring health care associated infections. To take responsibility for your own learning and development by recognising and taking advantage of all opportunities to learn in line with appraisal and supervision. Further Information for Postholder(s) We are a very dedicated, highly motivated, and dynamic team. We have a true MDT approach, and our team consists of a number of different disciplines working towards common goals for our service users. We offer a range of Psychological Therapies and Psychosocial Interventions and have a comprehensive training programme across the directorate. We have regular in-house training and clinical supervision with our Clinical Psychologists. We work very closely with service users and families, offering family interventions and therapy. We value service user involvement and encourage participation in service developments on many levels, including via our Peer Mentor system and our Service User representatives who assist us with recruitment. We pride ourselves on maintaining a strong staff wellbeing ethos where staff are encouraged to practice self-care strategies not only to keep themselves well and happy, but also to ensure that a high-quality service is consistently provided to service users and carers by staff who lead by example. The post holder is therefore advised smoking is not permitted within the hospital premises or grounds or whilst representing the Trust in the course of their duty.
