Carry out medicine reconciliation following hospital discharge and clinic letters Monitor patients requiring medication reviews or blood test monitoring Identify potential prescribing or medication safety issues and escalate appropriately Monitor the team email inbox regularly Maintain accurate patient medication records using clinical systems Liaise with GPs, pharmacists, community pharmacies, hospitals, and other healthcare professionals regarding queries Assist with searches and quality improvement work related to medicines management Support the implementation of prescribing safety alerts and NHS guidance Provide advice and support to patients regarding their medicines where appropriate Promote cost-effective and safe prescribing practices Ensure confidentiality and information governance standards are always maintained Update, monitor and review the monthly Clinical Safety Packs Handle function specific information, which may be sensitive, complex or confidential and appropriately recording, transferring and/or coordinating such information in accordance with the Data Protection Act; Caldicott Guidelines and the Confidentiality Code of Conduct To undertake, participate and share the outcomes of clinical audits to facilitate improvement and changes in practice To undertake learning and development to ensure the required knowledge and skills for the role To carry out other duties which are appropriate to the skills and competencies of the post holder and grade of the post as the priorities of the service chang
