Facilitation Facilitation of close MDT working across all sectors Leading on and coordinating practice-based and specialist MDT meetings Co-chairing of MDT meetings across the city with clinical leads Ensuring timeliness and focus of meetings, collation of patient names flagged for discussion, distribution of documents and action plans. Gathering relevant information to 'set the scene' prior to the MDT Completion of all administration including liaising with Adult Social Care to determine if any current involvement and if appropriate seeking feedback from named worker. To be the first point of contact for GP Practices and other providers to guide and assist in maximising effectiveness Proactively seek and recommend areas for improvement. Support General Practice with risk stratification and identification of new patients for discussion Organisation Ensuring there is a confidential, formal and consistent process for receiving and distributing patient information in a timely manner Encouraging all providers to submit patient information and ensuring these are discussed at the meeting. Agreeing meeting times/dates/locations and circulating information in good time. Coordinating non-clinical cross-service referrals Coordinating and distributing agreed actions and interventions Travelling between neighbourhoods Utilising the EMIS clinical system to capture and manage appropriate patent information.Meeting involvement Encouraging all providers to have an equal voice and facilitating input from non-vocal attendees. Encouraging all views to be considered Encouraging all attendees to be prepared for MDTs to maximise effectiveness Monitoring and reporting on outstanding actions Monitoring the identification of patients and encouraging nominations Ensuring fair participation and neutralising challenging views Support Support the clinical lead to chair meetings and ensure all attendees are aware of their agreed actions and the care plan that has been agreed going forward Solution focussed to address barriers Induct new starters into the MDT way of working Work with Practices to identify barriers to achieving full-service requirements and work with them to improve outcomes Collate and share good practice and learning outcomes with own team and the wider MDT Quality Record significant, adverse and near miss events via the appropriate systems Data cleanse of patients if deceased, moved practice, discharged from MDT Alert appropriate system leaders and managers on issues of quality and risk Assess own performance and take accountability for own actions, either directly or under supervision Contribute to the effectiveness of the team by reflecting on own and team activities, making suggestions on ways to improve and enhanced the teams performance Work effectively with individuals in other agencies to meet patients needs Effectively manage own time, workload and resources Monitoring Effectiveness Support with management of the Service Level Agreement requirements Reporting any concerns when the SLA is not adhered to Monitoring and reporting the frequency and attendance of key professionals at MDTs Monitoring and reporting the quality of MDTs and preparedness of providers to discuss patients Ensuring an appropriate number of patients are discussed in one meeting to maintain quality of discussions Analysis of relevant MDT data as required Identifying and reporting system coding issues and assisting resolution Communication Communicate effectively and appropriately with all members of the MDT Informing patients and their carers of the up-coming MDT and seeking consent Informing patients and their carers of care plans post MDT with clinical support if required Recognise peoples needs for alternative methods of communication and respond accordingly Proactively engage Practice staff and other professionals appropriately in the pursuit of the Coordinator duties. Relationships Build and maintain good working relationships with the following professional groups: GPs Practice Managers & practice administrative teams Community & PCN Frailty teams Social Workers District nurses Living well link workers Carers Mental health teams Any other person involved in patient care as part of MDT working Confidentiality In the performance of the duties outlined in this job description, the post-holder may have access to confidential information relating to patients and their carers, practice staff and other professionals.
