11. Job Summary The Practice Nurse provides safe, evidence based nursing care to the practice population. Working autonomously and as part of a multidisciplinary team, the postholder delivers chronic disease management, immunisations, screening, wound care, health promotion, and contributes to population health improvement. 2. Key Responsibilities Clinical Care Chronic disease management - diabetes, asthma, COPD, hypertension, frailty reviews. Immunisations & vaccinations - childhood, adult, travel, seasonal campaigns. Cervical cytology - screening in line with national guidance. Wound care and treatment room services - dressings, compression therapy, postoperative care. Clinical procedures - ECGs, venepuncture, spirometry, BP monitoring, NHS Health Checks. Assisting the GP with minor surgery Health promotion - lifestyle advice, smoking cessation, weight management. Long-term condition recall - proactive management using practice protocols. Professional Responsibilities Maintain accurate clinical records. Work within the NMC Code and scope of professional practice. Participate in audits, QI projects, and clinical governance. Support infection prevention and control (IPC) standards. Contribute to safeguarding of children and adults. Engage in CPD and revalidation activities. Team & Service Development Work collaboratively with GPs, ANPs, HCAs, pharmacists, wider MDT and admin teams. Assist in developing and reviewing clinical protocols. Support new staff and students. Contribute to population health initiatives and reducing health inequalities.
