We have a band 3 Community Support Worker position in our Community Response Team based at Walton Hospital, Chesterfield.
Our team provides vital care and support to patients in their own homes, particularly following a crisis or after discharge from hospital. This service ensures that individuals receive the appropriate care and assistance they need to recover and maintain their independence in a familiar environment.
We work collaboratively across a large geographical area and maintain close links with our other planned and rapid teams in North East Derbyshire and Chesterfield to deliver seamless, patient-centred care.

To provide holistic care to patients in their own homes, under the supervision of the appropriate registered professional.
To communicate information effectively
To report and take appropriate action regarding any change in patient condition or circumstance and communicate verbal and written information to all relevant integrated care team members
To maintain and record accurate health records electronically, in line with DCHS policies.
To participate as a team member in a wider integrated team, in continuous learning to develop your own knowledge and skills
To have a level of understanding with regards to the importance of safeguarding.
Respect the dignity of individuals.
Obtain consent when involving the patient, their carers/families/relatives in shared decision making.
To undertake all aspects of clinical duties and procedures which are documented using SystmOne
