The Advanced Nurse Practitioner (ANP) will work as an autonomous practitioner within the primary care team, delivering highquality, evidencebased care to patients. The post holder will assess, diagnose and manage patients with a range of acute, chronic and complex conditions, contributing to the delivery of safe, effective and patientcentred services. Clinical Responsibilities-Undertake comprehensive clinical assessment, diagnosis and management of patients presenting with undifferentiated, acute and longterm conditions. Provide independent prescribing in line with scope of practice and current legislation. Request, interpret and act upon appropriate investigations. Make referrals to secondary care, community services and other professionals as required. Manage sameday demand, urgent appointments and planned care clinics. Provide health promotion, disease prevention and patient education. Maintain accurate, timely and comprehensive clinical records. Professional Responsibilities Work autonomously within agreed clinical guidelines and protocols. Practice in accordance with the NMC Code of Professional Conduct. Maintain professional registration, revalidation and CPD requirements. Participate in clinical supervision, appraisal and peer review. Quality, Governance and Service Development Contribute to clinical governance, audit and quality improvement activities. Support the development and review of policies, protocols and pathways. Contribute to learning, teaching or mentoring of staff where appropriate. Promote a culture of safety, learning and continuous improvement. Team Working Work collaboratively with GPs, nurses, pharmacists, allied health professionals and administrative teams. Support integrated working across the Primary Care Network. Contribute positively to team effectiveness, communication and morale. Other Duties Undertake any other reasonable duties consistent with the role and level of responsibility. Be flexible to meet service needs.
