Detailed Responsibilities The post-holder will: Champion trauma-informed and relationship-based practice across neighbourhood services Champion workforce transition to a trauma-informed approach for all service users Develop strong partnerships between health, social care and voluntary sector organisations Support people with complex life experiences to access appropriate services and community support Work closely with social prescribers, peer workers, community connectors and multidisciplinary teams Help embed inclusive and culturally sensitive approaches across services Contribute to service redesign focused on prevention, wellbeing and community resilience Support community engagement and co-production with people who have lived experience Create and actively sustain accountability, scaffolding and implementation support for embedding a trauma-informed and responsive approach across the organisation and take a joined-up approach with partners. Partnership & Collaborative Working Work closely with VCSE partners, community organisations and local services Facilitate multi-agency discussions and collaborative planning Strengthen connections between statutory services and grassroots community support Promote person-centred, strengths-based approaches Key Outcomes Workforce training, transition and development to a trauma-informed practice Improved engagement and trust with individuals facing complex challenges Put in place improved systems for service users presenting in crisis. Stronger partnerships between statutory and voluntary sector services Increased staff and volunteer confidence in trauma-informed approaches Enable greater community voice and participation in service development Working Relationships Voluntary, Community and Social Enterprise (VCSE) organisations Community groups and peer support networks St Austell Primary Care and St Austell Integrated Neighbourhood Teams Community Social Prescribing teams Mental health services (Cornwall Foundation Trust) South West Ambulance Service Royal Cornwall Hospital Adult Social Care and housing providers People with lived experience (service users) and community representatives
