As part of the integrated service provision the post-holder will work within the Urgent CommunityResponse(UCR)servicetoassistinthepreventionofinappropriatehospital admissionsandtheprovisionofhome-basedtreatmentinlinewiththeoverallobjectives of the service. The post-holder will carry out personal care and assist the senior healthcare professional with acute care needs.



To support the UCR team in the delivery of safe, effective care to patients in their own homesandcommunitysettings.Thepostholderwillworkalongsidetheteamto provide practicalsupport,andcontributetoloneworkingsafety,ensuringvisitsarecarriedoutin line with Trust policies and risk assessments.

Thepostholderwillbeguidedbyamoreseniorhealthcareprofessionalatalltimesunder the Guidance for the Delegation of Care and Treatment from Registered Nurses, Nurse Associates and Allied Health Professionals to BCHC employed non-regulated support staff policy (CH567).

· Carryoutcareandinterventionsunderthedirectionofaregisteredprofessional, including manual handling of patients.

· Workinthecapacityofachaperoneinlinewithcompetencies.

· SupportUCRteamduringhomevisitsforsafetyorriskmanagement.

· AssistinmaintainingpersonalsafetybyaccompanyingClinicians duringvisitsto reduce lone working and following risk assessment procedures.

· Prepareequipmentandenvironmentsforpatientcare,ensuringcleanlinessand infection prevention standards are met.

· Observeandreportanychangesinapatient’sconditiontotheUCRclinician

o promptly.

· SupportUCRteamduringvisitswherethereareknownrisks(e.g.challenging behaviour, environmental risks, safeguarding concerns).

· Reportincidents,nearmisses,orsafetyconcernspromptlyandaccurately.

· Ensuretimelyinputtingofrelevantactivitydatawithinthedesignatedtimeframeagreed.

· Acttosafeguardthepatienttakingemergencyactionwherenecessary,reporting any potential or actual risks to the patient to a senior team member immediately and following the safeguarding policy.

· Respectthedignity,wishesandbeliefsofpatientsandcarers;involvethemin shared decision making; and obtain patient consent prior to all interventions. Patients may have barriers to understanding.

• Communicate effectively both verbally and in writing with colleagues and stakeholders,utilisingelectronicmethodsofcommunicationwithinPolicyand Procedures set out by Information Governance.
• · Attendmandatorytraining,includingloneworking,conflictresolution, safeguarding, and infection prevention.
