Summary
This is an exciting opportunity to join our Care Leavers Team, supporting young people aged 16–25 across Cheshire East as they transition into adulthood. Working within corporate parenting principles, the role focuses on empowering care leavers with the skills, confidence, and support they need for independence. Through collaboration with a wide range of professionals - including social workers, housing providers, mental health practitioners, and employment support teams - you’ll play a vital part in shaping positive futures.

About the role:
Provide guidance and practical support to care leavers aged 16–25, helping them prepare for independence.
Work in partnership with professionals across housing, health, education, and employment services.
Co-produce Pathway Plans with young people and key individuals in their lives.
Maintain regular contact with care leavers and ensure access to the Cheshire East Care Leavers Local Offer.
Deliver duty support services alongside other Personal Advisors and keep accurate, timely records.

About you:
You hold an NVQ Level 3 in Advice and Guidance or Health and Social Care.
You’ll bring excellent organisational skills, strong personal resilience, and a passion for improving outcomes for young people.
You have experience working with diverse backgrounds and are committed to promoting positive outcomes for all care leavers.
You’ll enjoy building relationships and working in partnership with other agencies.
You have strong interpersonal skills and thrive in a relationship-based practice approach.
