JOB DESCRIPTION Practice Nurse Reports To Nursing services manager PURPOSE OF THE ROLE To work as a Practice Nurse within Belvoir Health Group, taking responsibility for assessing, planning and delivering nursing care to patients with a broad range of healthcare needs and long-term conditions. The post holder will provide high-quality, evidence-based and accessible nursing services to patients, ensuring clinics are managed efficiently and delivered in a timely manner wherever reasonably practicable. All duties and responsibilities will be carried out in accordance with Practice protocols, professional standards and guidance developed by the Nursing Team, in conjunction with the Nursing Services Manager. The Practice Nurse will work collaboratively with Senior Nurses, Site Team Leaders and the wider multidisciplinary team to ensure the effective delivery of patient care, including patient recall and review systems, rotas and service planning. MAIN DUTIES AND RESPONSIBILITIES Clinical Duties Provide treatment room nursing services including: Wound care and wound management Venepuncture ECGs Routine injections Urinalysis Work collaboratively with GPs and Non-Medical Prescribers to organise medications, wound dressings and ongoing plans of care. Undertake travel health assessments and provide basic travel vaccinations, malaria prophylaxis and travel health advice, referring for specialist advice where appropriate. Provide contraceptive and sexual health services, including: Contraceptive advice Coil checks Administration of Depo-Provera injections in accordance with protocols Ensure patients are appropriately monitored according to their chosen contraception treatment pathway. Provide cervical screening services in line with National Policy and NHS recall systems. Undertake routine swabs as requested by GPs or Advanced Clinical Practitioners. Deliver childhood and adult immunisations in accordance with the national immunisation schedule. Carry out screening examinations including: NHS health checks Undertake home visits on behalf of the Practice where appropriate and requested by the GP or Nursing Services Manager. Long-Term Condition Management Support the ongoing management of patients with long-term conditions including: Coronary heart disease Hypertension Diabetes COPD Asthma Participate in the maintenance of disease registers and patient recall systems. Work collaboratively with GPs and the wider clinical team to deliver evidence-based chronic disease management in accordance with agreed protocols and individual competencies. In cases of co-morbidity or complexity, work closely with the Nursing Services Manager to ensure coordinated patient care. Professional Responsibilities Maintain accurate, clear and contemporaneous clinical records, ensuring all consultations, communications, specimens and prescriptions are managed safely and appropriately. Ensure nursing care remains evidence-based, effective and evaluated appropriately. Participate in clinical audit, service improvement initiatives, research activities and quality assurance programmes. Represent Belvoir Health Group at PCN and ICB meetings as delegated by the Nursing Services Manager. Support, supervise and assist in the training of Health Care Assistants, Phlebotomists and Nursing Students as required. Attend mandatory training, Practice meetings and continuing professional development activities to maintain professional registration and clinical competence. Adhere fully to the Practice complaints procedure and ensure patient concerns and complaints are managed appropriately and professionally in line with NHS guidance. Ensure clinical rooms are maintained to high standards of cleanliness and safety, including participation in Infection Prevention & Control and Health & Safety procedures and audits. Undertake any other duties appropriate to the role, competencies and experience as delegated by the Nursing Services Manager. LEARNING AND DEVELOPMENT The post holder is expected to maintain and develop professional knowledge and skills through continuing education, training and development in line with NMC requirements and Continuing Professional Development (CPD) standards. The Practice Nurse will also be expected to develop interpersonal skills, emotional intelligence and leadership behaviours in line with the culture and values of Belvoir Health Group. The post holder will be required to travel between all Belvoir Health Group sites and may occasionally be required to travel to other PCN locations as part of the role. The post holder is expected to work in accordance with the policies, procedures, values and standards of Belvoir Health Group.
