Summary

Provide high-quality, patient-centred care, including prevention, screening, diagnosis, treatment, management of chronic conditions, and health promotion Develop and implement care and treatment plans in partnership with patients, in line with Practice protocols and national guidance (e.g. NICE) Undertake face-to-face, telephone, and video consultations, including home visits where clinically appropriate Participate in on-call duties and triage, supporting effective management of urgent and same-day demand See and consult with patients in a timely and efficient manner, helping to minimise waiting times Ensure appropriate follow-up and continuity of care for patients Issue and review acute and repeat prescriptions in line with practice policies and prescribing guidelines Work collaboratively with the multidisciplinary team and liaise with external services involved in patient care Assess, investigate, and refer patients appropriately, in line with current clinical guidance Manage laboratory results, clinical correspondence, and administrative tasks promptly, ensuring clear documentation and follow-up actions Provide clinical cover for GP colleagues during periods of leave Ensure the effective and responsible use of NHS resources Maintain accurate, contemporaneous, and high-quality electronic patient records in line with practice policies Contribute to audit activity, QOF requirements, and quality improvement initiatives Prescribe cost-effectively, following generic prescribing where appropriate and adhering to formulary, commissioning, and national guidance Maintain excellent communication with patients and carers to support safe and effective care delivery
