Neuro navigation is a well established role in City and Hackney supporting all patients who require a specialist and/or complex neuro rehabilitation pathway, from advice and signposting within specialist rehabilitation, supporting discharge and longer term complex case management working collaboratively with continuing healthcare and providing expert clinical advice to the adult community rehabilitation team and specialist neuro rehab services within the Homerton.

The neuro navigator is expected to understand complex rehabilitation pathways accessed by clients with a neurological diagnosis and long term care services; including clients with deteriorating neurological conditions, as well as single neurological events.

You will provide expert clinical opinion on and monitor referrals and admissions for level 1, 2a, 2b inpatient rehabilitation provision, community and out of borough placements. You will work in partnership with other health and social care providers, continuing healthcare and promote an integrated approach to the management of progressive and non progressive neurological conditions.
