Crynodeb
To lead on the assessment and re-assessment of patients and carers receiving home nursing and volunteer support. To coordinate and manage the Hospice at Home Day Night Nursing Team, acting as supervisor and co-ordinator for the Day Night Nursing Service in the absence of the Head of Clinical Services. Assist the Head of Clinical Services to develop and support nurses and the wider team through an effective appraisal and development review process, identifying learning needs and provide training opportunities. Participate in direct provision of patient care and liaise with members of the multi-disciplinary team and palliative care team in order to achieve this.
