Crynodeb

The Advanced Clinical Practitioner (ACP) will provide expert clinical care within General Practice, working autonomously and collaboratively with the multidisciplinary team to assess, diagnose, treat, and manage patients presenting with undifferentiated and undiagnosed conditions. The post holder will deliver high-quality, evidence-based care across acute, chronic, and preventative health needs, supporting the practice in improving access, patient outcomes, and service delivery. Main Duties and Responsibilities Clinical Practice Undertake advanced clinical assessment of patients with acute, undifferentiated, and complex presentations. Diagnose and manage patient conditions using clinical judgement and evidence-based practice. Develop and implement treatment and management plans. Prescribe medications independently within scope of practice and current legislation. Request, perform, and interpret diagnostic investigations. Provide same-day urgent care appointments and telephone triage. Manage patients presenting with minor illness and minor injuries where appropriate. Undertake home visits when required. Refer patients appropriately to secondary care, community services, and allied healthcare professionals. Support management of long-term conditions and preventative healthcare. Maintain accurate, contemporaneous clinical records using practice clinical systems. Professional Responsibilities Practice within ACP competencies, professional code of conduct, and organisational policies. Maintain professional registration and comply with revalidation requirements. Participate in clinical supervision, reflective practice, and appraisal. Maintain and develop advanced clinical knowledge and skills through continuous professional development (CPD). Contribute to clinical governance, audit, and quality improvement activities. Identify and report clinical risk, safeguarding concerns, and significant events appropriately. Team Working Work collaboratively within the multidisciplinary team including GPs, nurses, pharmacists, paramedics, and administrative staff. Support training and mentorship of junior staff and students where appropriate. Participate in practice meetings and service development initiatives. Promote integrated working across the Primary Care Network and community services. Communication Communicate effectively and sensitively with patients, carers, relatives, and colleagues. Provide health education and promote self-management where appropriate. Ensure confidentiality is maintained in accordance with GDPR and information governance policies.
