Job Statement The Practice Nurse delivers highquality, patientcentred nursing care within the primary care environment. The role combines autonomous clinical practice with collaborative multidisciplinary working, supporting longterm condition management, preventative health interventions, urgent care, and treatmentroom services. The postholder contributes to safe, effective, and compassionate care while supporting the operational and clinical priorities of the practice and wider PCN. Key Responsibilities Clinical Care Provide evidencebased nursing care to patients across the lifespan. Deliver longterm condition management (e.g., asthma, COPD, diabetes, hypertension, CVD). Undertake cervical cytology, immunisations, wound care, ear irrigation, ECGs, and phlebotomy. Support urgent care through assessment, triage, and treatment planning within scope. Provide health promotion, lifestyle advice, and selfmanagement support. Interpret clinical findings and escalate concerns appropriately. Maintain accurate, timely clinical records in line with NMC and organisational standards. Professional Leadership & Governance Work in accordance with the NMC Code and professional standards. Adhere to safeguarding, infection prevention and control, medicines management, and clinical governance frameworks. Participate in clinical audits, quality improvement, and service development. Maintain uptodate competencies and mandatory training, including safeguarding levels 13 as required. Contribute to the development of clinical pathways, protocols, and best practice guidance. Teamwork & Collaboration Work closely with GPs, ANPs, HCAs, ARRS roles, and administrative teams. Support induction, supervision, and development of junior nursing staff and HCAs. Participate in MDT meetings, case discussions, and PCNwide initiatives. Provide clinical advice and support to colleagues within scope of practice. Patient Experience & Communication Build positive, trusting relationships with patients and carers. Communicate complex or sensitive information clearly and compassionately. Promote shared decisionmaking and personalised care. Ensure patients understand treatment plans, medications, and followup requirements.
