Summary
We are expanding into West Lancashire and want an experienced care co-ordinator that is capable of taking work on and has links with Lancashire County Council, etc.
You will be heading up your own area and we will expect you to: (1)Take on the work
(2) Care plan and risk assess (3) Allocate to staff (4) Manage your team (5) manage your on call and cover for your area.
