KEY RESPONSIBILITIES Clinical To gain consent from patients and relatives by sharing clear, easily understood information relating to any delegated interventions. To undertake and maintain patient and professional confidentiality at all times To undertake initial assessments independently or with other members of the team as required. To identify patient needs and interpret assessment information and plan and progress intervention using judgement skills to recommend the best course of action in consultation with senior members of the team. To accept responsibility for designated caseload working independently to promote independence and meet the needs of the service user including providing advice on wellbeing and safety, to assess and provide adaptations and refer onto other agencies should the patients needs be out with the role of the practitioner. To work in a variety of settings that may include clients own homes and care homes. Take an active role in management of patients referred to the service and supporting the therapists and nurses in on Home Assessment Visits To deliver an integrated service by effectively liaising with other care providers providing information of health and social needs as appropriate, working within legislation, policies and procedures and upholding core values. To ensure effective, compassionate communication and exchange of information with clients/carers through direct contact, written communication and telephone contact. Utilise appropriate resources in the event of communication difficulties being present. To independently plan, prepare and carry out the appropriate level of rehabilitation and or nursing tasks and evaluate progress in line with treatment and care plans plans, with support from senior members of the team. To liaise with multi-professional teams and other agencies to ensure good outcomes for clients. To actively consider all aspects of risk management whilst carrying out clinical and non-clinical duties. Maintain accurate patient records and documentation as per organisation policy. To undertake administrative, clerical and housekeeping duties to promote good practice and organisation within the team. To take referrals into the service for triage by clinicians and therapists and/or signpost to other appropriate services. Provide support to the Triage nurse as required. To undertake any other duties that may be reasonably required by clinicians. Educational Assist in the induction, training and education of other staff and students. Able to demonstrate own role to new or less experienced staff. To be involved in appraisal systems, clinical supervision, and be active in identifying aims and objectives for personal development in line with City care Policy. Active participation in appropriate In-Service training programmes delivered by the team. Ensure compliance with all mandatory and service specific training. Maintain an ability to safely and competently assist with the handling and moving of patients when this is required therapeutically, including the safe use of all relevant manual handling equipment, plus wheelchairs and electric beds. To educate clients and their carers in the safe and correct use of equipment. Organisational To work flexibly to meet the needs of the service Exercise good personal time management, punctuality, consistent reliable attendance, and day to day management of own delegated workload and maintain a professional image Aware of own role and scope of practise and the contribution within the team, and be able to seek supervision and support from a Therapist/ Nurse when necessary. Contribute to the collection and collation of patient information and data as required by the service.
