Working Hours: 37 hours per week Monday-Friday 9am-5pm, with the potential to work evening shifts when needed

Salary: £25,877.80 - £34,240.03 per annum

About the role:

The role of the resettlement worker is to drive forward move on outcomes and reduce the length of time a person resides within the service after completing their recovery program and are ready to move onto the next stage of their journey into independence.

Planning/Service Delivery

Actively engage with key partners and stakeholders to source/improve move on options for people leaving our service.
Establish links with new potential landlords and letting agencies.
Ensure all clients are registered with Sheffield council housing within the first month of residing at The Greens.
Explore options and apply for funds for rent in advance payments and deposits for private sector tenancies.
Create and facilitate a resettlement program for people using the service that will provide them with tools and information that will support them back into living independently in the community.
Provide transitional support to people leaving the service. This may include support to attend property viewings, support during tenancy sign up, setting up utilities and support to familiarise them with their new local area.
Facilitate a housing related drop-in group for recently departed individual’s to access triage support with setting up utilities, benefit queries and general transitional support
Work with individual’s to co-produce personalised resettlement plans.
Apply for grant funding that may support with the purchasing of start-up items such as carpets, furniture and white goods.
Performance

Provide a quarterly report of move-on outcomes to the service manager.
Work collaboratively to meet service KPI’s.
Attend meetings with stakeholders and key partners and feedback data/information to service manager.
Development and engagement

Represent Waythrough in a professional manner and network, build and maintain effective professional relationships with internal and external professionals and stakeholders including service users, staff teams, commissioners and other service providers.
Undertake any training identified as relevant to the role
About you:

Level 3 Certificate in Housing Practice or Housing Services (Desirible)
Knowledge and experience within health and social care sector (substance misuse, mental health, housing etc)
Experience of working within either supported housing or a housing management position
Knowledge of housing legislation and the barriers that complex individual’s may face with access to accommodation
Ability to develop relationships with professionals at all levels, including senior registered professionals and statutory services
Ability to manage multiple workstreams working to tight deadlines
Excellent written and verbal communication including report writing
