Crynodeb
The role will be 6-8 sessions to be agreed and will be over both sites to undertake a variety of duties including surgery consultations, telephone consultations, home visits, processing of prescriptions and dealing with queries/paperwork (docman) in a timely fashion. The successful candidate will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all patients. Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation. Assessing the health care needs of patients with undifferentiated and undiagnosed problems. Recording clear and consultation notes. Screening patients for disease risk factors and early signs of illness. Developing Care Plans & Health Education Other responsibilities include: Awareness of relevant practice policies and procedures Managing and ensuring all training is up to date for Safeguarding, Data Protection, Health & Safety Attending surgery meetings & training
